We operate an Intensive Outpatient Program (IOP) and provide Outpatient (OP) and Opioid Health Home (OHH) services that integrate clinical care, peer support, and care coordination to help individuals build meaningful, sustainable lives in recovery.
Position Summary
The Patient Navigator/Care Coordinator at Courage Center is a position in our Opioid Health Home (OHH) and Outpatient services. This role combines administrative coordination with peer recovery support, utilizing lived experience to enhance client engagement, streamline operations, and support retention in services. This role plays a critical role in ensuring clients successfully navigate services while also supporting administrative tasks, documentation, communication, and client outcomes. The Patient Navigator/Care Coordinator works closely with the clinical supervisor and medical staff to support a person-centered, recovery-oriented system of care.
Key Responsibilities
- Serve as a consistent, welcoming point of contact for clients—supporting engagement, connection, and follow-through throughout their care
- Provide front-end administrative support including scheduling, phone coordination, and client check-ins
- Assist with intake and onboarding processes, including completion of paperwork, consents, and releases of information (ROIs)
- Help individuals navigate services, appointments, and community resources, reducing barriers to care (e.g., transportation, system navigation, access to services)
- Coordinate communication between clients and providers, including clinicians, nurse care managers, prescribers, and external partners (e.g., DHHS, probation, medical providers)
- Support scheduling and coordination of counseling, groups, medication management, and nursing services
- Complete accurate and timely documentation, notetaking, and recordkeeping in BestNotes (EHR)
- Support tracking of OHH and outpatient service delivery, including contacts, group participation, care coordination activities, and UA tracking as directed
- Conduct and document urine drug screens (UAs) with professionalism, respect, and adherence to agency policy
- Assist with chart organization, audit preparation, and maintaining compliance standards
- Work closely with the Clinical Supervisor, Nurse Care Manager, and OHH team to support integrated, whole-person care
- Participate in team collaboration, program development, and ongoing growth of services
- Model recovery-oriented behaviors, healthy coping strategies, and effective system navigation
- Utilize lived experience to engage and support individuals participating in Courage Center’s OP and OHH programs
- Provide peer-based support, encouragement, and mentorship to individuals in recovery
- Support client engagement in services, including attending scheduled appointments, groups, and OHH contacts
- Reinforce a person-centered, multiple-pathways approach to recovery
Qualifications
- Lived experience in substance use and/or mental health preferred
- Recovery Coach Certification, Certified Peer Support Specialist (CRSS/MHRT/CSP), and/or Alcohol and Drug Counselor Assistant (ADCA) preferred
- Must be certified or willing to obtain one or more of the above credentials within six (6) months of hire
- High school diploma or equivalent required, additional education in behavioral health or human services preferred; students encouraged to apply
- Experience in administrative support, healthcare, or behavioral health settings preferred
- Strong organizational, communication, and multitasking skills
- Ability to maintain confidentiality and appropriate professional boundaries
- Proficiency with computers, EHR systems, and Microsoft/Google platforms
